Registration Procedure
**You must follow these directions exactly, otherwise your registration will not be processed, and your team will not be allowed to participate**
Registration for the Boys' Pro Division has been closed for both sports
- The team coach must electronically submit this Registration Form
- The team coach must Download this Waiver Form and have it filled out completely by each player.
- A clear and legible photocopy of a picture ID which shows the name and birthdate of each player must also be submitted. This rule applies to all age groups including Pro division
- The registration fees must accompany the waiver forms and picture IDs in one package via Certified Mail. This must be received within 5 days of submitting the electronic application.
- No electronic applications will be accepted after March 1st, 2006. NO EXCEPTIONS TO THIS RULE
- Any fees, waiver forms, picture IDs will not be accepted after March 3rd, 2006. NO EXCEPTIONS TO THIS RULE
